Having trouble with our forms? These links have been tested with both IE 4+ and Navigator 4+ and are displaying and downloading properly. If you are having trouble viewing or printing these forms, please ensure that you have Adobe Acrobat Reader, version 4 or higher, properly installed on your computer. (Acrobat Reader is a free download for Mac or Windows from Adobe Systems Incorporated.) To view PDF files on-line, make sure that your browser preferences are set to view PDF files using either the Acrobat Viewer plugin or the Acrobat Reader application (set in preferences under "file helpers" in Internet Explorer or under "applications" in Netscape Navigator). If you want to print the form you are viewing, be sure to click the print button on the toolbar that comes up with each Acrobat document, and NOT the regular browser print button at the top of the browser screen. If you prefer to download the files, click and hold on the link until your browser launches a pop-up window. Select "Download link to disk" (Internet Explorer) or "Save link as..." (Netscape Navigator), and follow the directions to save the PDF to your computer. You may then open, view and print the form, using Acrobat Reader, at your convenience. |
